You can request Telemedicine / Telehealth / Video consultation with Doc Gautham’s Neuro Centre* through this page. Please Note: As per the Telemedicine Practice Guideline of India, seeking consultation through this page implies that you have read and consented to our Terms and Conditions of Telehealth / Tele-Medicine. The step-by-step process for Telehealth / Telemedicine consultation is given below.

*Please call 95661 33660 (between 10 am and 3 pm IST on week days) for charges and other clarification. 

We have tried to make the telemedicine / telehealth consultation process as smooth as possible. However there are certain processes that we have to follow as per the Telemedicine Practice Guidelines.
  1. Please fill in and submit the form (on the right) to make an appointment.
  2. Once you have submitted your request for the telemedicine / telehealth consultation, you will receive an email confirming receipt of your request for telemedicine / telehealth consultation. 
  3. The email will also contain details of payment that requires to be made along with instructions and requirements from you as per the Telemedicine Practice Guidelines of India.
  4. You will be required to submit a photo ID at the time of payment.
  5. Payment is to be made to the bank account detailed below.
  6. On receipt of the payment and photo ID, an appointment will be fixed for the Telemedicine consultation and you will be intimated of the same through email. 
  7. The telehealth / telemedicine consultation will be through WhatsApp video call or any other mode that will be communicated to you.
  8. If you do not get a confirmation email within two working days of submitting the telemedicine consultation request please call +91 95661 33660 between 10 am and 3 pm IST on week days.

*Includes Dr. Gauthamdas Udipi and / or doctors, healthcare personnel and staff of Doc Gautham’s Neuro Centre.
